Goal Setting

Once the initial meeting is held, our team will walk you through your plan to start working towards your NDIS goals. These are highly important meetings and the team will be able to support you in setting achievable milestones in working towards goal outcomes.

Our friendly Support Coordination team will meet with you in your own home, or wherever you are most comfortable, to discuss your NDIS Support Coordination needs. They will be able to walk you through your NDIS plan and will be able to assist you with making a plan on how best to utilize all of your funding appropriately.

During your NDIS plan period our Support Coordination team will continue to support you along your NDIS journey. This will be personalised to your individual needs and will be done regularly to give you more opportunities to “fine tune” your NDIS experience. As life changes every day, so too will your needs. We will be standing beside you the whole time offering our support and guidance.
